Riverhead Care Center offers a specialized rehabilitation program for those patients who would benefit from continuous telemetry monitoring while undergoing therapy.

Upon admission each patient is evaluated to determine eligibility for our Cardiac Telemetry Monitoring Program. The program is overseen by our Director of Cardiac Rehabilitation who is a board certified cardiologist.

Therapy sessions are administered by licensed therapists and supervised by Advanced Cardiac Life Support (ACLS) Certified Registered Nurses. State of the art hospital grade telemetry equipment is used to closely monitor each patient's heart rate, cardiac rhythm, vital signs and response to exercise.

Our Cardiac Telemetry Program coordinates with other disciplines within our facility including dietary, social services, and therapeutic recreation to assist patients in reaching their goals.

Candidates admitted to our program include but are not limited to those with:

• Valve Replacements
• Coronary Artery Bypass Grafts (CABG)
• Arrhythmia including those with
• Atrial Fibrillation
• Syncopal Episodes
• Congestive Heart Failure

Riverhead Care Center is equipped with the latest life saving technology including an Automated External Defibrillator (AED). Our experienced and dedicated staff work with each patient to provide a comprehensive and individualized heart health care therapy program.
